This Plan is intended to provide specific information necessary for effective site implementation of the Company Safety Policy. More detailed safety information is contained in the Handbook. This Plan shall be completed prior to the start of work by the Project Supervisor for each project involving three (3) or more employees of more than one (1) weeks duration.

o Safety Policy Orientation and Acknowledgement

• Require all personnel who work on the project (irregardless whether Aldrich + Associates’ employees or those of a subcontractor) to review the Site Specific Safety Plan contained in this manual and acknowledge this review by signing the Employee Orientation Checklist – Safety form. See tab 10A of this Site Specific Safety Plan for blank forms.
• Place a copy of the signed Employee Orientation form behind Tab 10A of this Site Specific Safety Plan and provide a copy to the office. For subcontractor employees on larger projects, you may file them in a separate notebook tabbed for each subcontract performing work on site.

o Safety Meetings

• Safety meetings are to be held at least once weekly. Reference information in the Safety Meeting Topics (White Three-ring Notebook) for discussion topics.
• The Safety Representative will post in a central location accessible by all project participants notification of Safety Meetings date and time. A form for this purpose is contained behind Tab 10H of this Site Specific Safety Plan.
• All Aldrich + Associates employees must attend each safety meeting. One acceptable method to conduct a safety meeting is to select a topic from the Safety Meeting Topics notebook, make a copy of the topic sheet and return the original to the Safety Meeting Topics notebook. After the Safety Representative has reviewed the information from the topic sheet with attendees at the safety meeting, require that everyone sign either the bottom or back of the topic sheet. Place the signed topic sheet behind Tab 10B and provide a copy to the office.

1. Hard hats will be worn at all times in the construction area.
2. Safety glasses will be worn at all times in the construction area. Face shields or respirators will be worn when a particular operation requires them.
3. Shirts will be worn at all times. This may save you from sunburn, acid and steam burns, weld splatters or cuts.
4. Suitable work shoes or boots in good repair will be worn at all times. No sandals or tennis shoes will be acceptable on the job. Safety shoes are recommended.
5. Gloves will be worn while operating stationary grinding, buffing or other rotating machinery.
6. Other appropriate Personal Protective Equipment will be worn as required.
7. No one will be allowed on the jobsite while under the influence of alcohol or narcotics, or while in possession of such.
8. Fighting, creating a disturbance or horseplay is not permitted on the jobsite.
9. Riders are prohibited on or in any moving vehicles that are not designed for passengers. No one but the operator will ride on a lift truck. Riding material hoists, crane loads, headache balls or the hook will not be permitted.
10. While lifting material, keep back straight, bend knees, and lift with the legs. If the load is too heavy, get help.
11. Do not walk or work under suspended loads.
12. Do not leave an opening unprotected or uncovered. Provide a cover or guardrail and toeboards.
13. Do not lean against safety line or guardrails.
14. Tie-off or anchor all ladders.

Safety Disciplinary Policy

1. Possession or use of intoxicating beverages or drugs in the workplace.

1. Cutting of wood or metals or grinding of concrete surfaces could result in airborne particles being embedded in an eye. Eye protection such as safety glasses with clip-on or slide-on side shields or face shields or goggles shall be worn at all times.
2. Certain activities could result in a head injury from falling or flying objects or from working in or around scaffolding. A hard hat shall be worn at all times when these types of hazards are present.
3. Certain activities could result in a foot being struck by falling objects, rolling objects, a piercing or cutting injury or an electrical hazard. Appropriate footwear shall be worn at all times.
4. Certain activities could result in hand exposure to harmful substances, severe cuts or abrasions, punctures, chemical or thermal burns or harmful temperature extremes. Appropriate hand protection such as gloves, etc will be provided when required by the activity.
5. Certain activities could result in the breathing in of air contaminated with harmful dusts, fogs, exhaust fumes, mists, gases, smokes, sprays or vapors. Appropriate respirators or dust masks shall be provided when required by the activity.
6. t is Aldrich & Associates policy to maintain a clean and safe project site at all times. Toward that end, we will vigorously enforce our Housekeeping and related Clean up policies. Reference information under tab a for specific requirements.
7. Certain activities could involve working in trenches or excavations deeper than 4’ that will require protection from cave-ins. Follow guidelines contained in tab b.
8. Certain activities could result in personnel working in or around rebar or other vertical elements that could result in serious injury should someone fall on or around these elements. Aldrich + Associates’ policy is to prevent injury from these vertical elements by covering them with suitable protective measures or rebar caps. Reference information under tab c for specific requirements.
9. Certain activities will involve the use of electricity for power tools and equipment as well as working conditions that will require adequate temporary lighting. Reference tab d for specific requirements that will be followed for this project.
10. Certain activities may result in the potential for electrical shock from equipment being installed. Equipment such as motor control center, motor starters and disconnects will be locked out when work is being performed on equipment

1. Housekeeping:
   A. Avoid accumulation of flammable rubbish and waste materials.
   B. Remove trash from inside buildings and away from buildings daily.
   C. Under no circumstances is wood sawdust or shaving to be used as an absorbent for spilled flammable liquids or petroleum lubricants.
   D. Burning of rubbish is prohibited.
2. Refueling of Equipment:
   A. All gasoline powered equipment will be refueled outside and clear of structures, with engines shut off. Gasoline powered equipment will not be allowed on building roofs.
   B. Gasoline powered equipment such as air compressors, hoists, pumps, etc. will be located so that the exhausts are well away from combustible material.
   C. Electrical or air driven equipment will be used underground, under buildings, in tunnels or on building roofs.
3. Heating Devices:
   A. Open flame devices will not be used in areas with flammable materials.
   B. All open flame devices and furnaces used in construction work will have an attendant unless equipped with combustion safety controls.
   C. All empty propane, acetylene, oxygen and butane tanks will be moved from buildings, identified as empty (MT) and will be stored and secured in an upright position in an approved area.
   
4. Welding:
   A. A welding permit issued by Aldrich + Associates required prior to the start of any welding operation except in designated welding areas. A welding permit will not be issued in the event that the fire protection system is impaired in the area of the request.
   B. All welding operations will have an approved fire extinguisher at the site, as appropriately determined by Aldrich + Associates.
   C. No welding or open flame devices will be operated within 50 ft. of any spray painting or use of any substance that may produce flammable vapors.
   D. All combustible materials which cannot be moved a safe distance from welding operations will be covered with approved, non-combustible (nonasbestos) blankets or non-combustible rigid barriers for protection from sparks.
   E. Radiation protection from arc welding operations will be provided for all individuals.

1. Attendance is mandatory for all Aldrich employees and all subcontractor’s employees not conducting their own Weekly Safety Meetings as confirmed by the Safety Person.
2. Special Safety Training prior to the start of a new activity or as the result of a need raised by the Safety Committee or a “near-miss”
3. Individual Training upon request from an employee to educate on the specific issues related to an unfamiliar activity.

1. he safety supervisor will be responsible for all safety training. Where required to supplement knowledge, additional consultants may be involved as needed to provide training.

1. From time to time, the safety supervisor or other Aldrich + Associates personnel in authority may require demonstration of a particular safety issue. An illustrated lack of knowledge related to a safety issue will result in supplemental training.
2. If any employee refuses to follow safety practices for which they have been trained, that employee will be terminated from employment with Aldrich & Associates and not eligible for rehire for a period of at least one year.

1. Work activities that could generate air-borne particles or liquids.
   

   o Wear eye protection such as safety glasses with clip-on or slide-on side shields meeting ANSI Z87.1-1989.
   o Wear face shields or goggles meeting ANSI Z87.1-1989.
   o Either safety glasses, goggles or helmets will be provided by Aldrich + Associates. Training in the proper use of this equipment will be provided by the safety supervisor.
   o Prescription protective eye wear (except as part of a full face piece or hooded respirator) will be provided by the employee as a prerequisite to entering upon an Aldrich & Associates jobsite.

2. Work activities that could result in being hit by falling or flying objects or are conducted under or around scaffolding or other overhead structures.

   o Wear a protective helmet (hard hat) meeting ANSI Z89.1-1986.
   o Newly hired Aldrich + Associates employees will be provided a hard hat upon starting work at the jobsite. These will be worn at all times when an employee is performing work on an Aldrich + Associates jobsite. If a hard hat is lost or stolen, the employee will be reissued a replacement hard at the cost of replacement.

3. Work activities that could result in feet being hit by falling objects, rolling objects, piercing or cutting injuries or electrical hazards.

   o Wear appropriate footwear meeting ANSI Z41-1991.
   o Appropriate footwear shall be defined as those made of leather or other equally firm material. The soles and heels of such footwear shall be of a material that will not create a slipping hazard. Shoes made of leather or other firm materials that have soft athletic-type soles which would protect employees from foot injuries and at the same time, provide soft and firm footing while working under specialty requirements or with specialty materials are acceptable if meeting safety shoe requirements established by OSHA or ANSI. Footwear that has deteriorated to a point where it does not provide the required protection shall not be used.
